how to Apply for assistance

(All Applications must be filled out in person)
The Forgotten Child Fund application center is located at 1715 E. Main St and will be taking applications from November 1, 2023, through December 13, 2023. Office hours are Monday - Friday, 10:00 am - 2:00 pm. See below for a list of items you will need to bring with you when you apply.   If you need more information on applying, contact us at 423-697-0031.
Eligibility:
1.  Have to live in Chattanooga, TN or surrounding cities or hear US 101.
2.  Children must be between 1 day old or 16 years old.  (We will assist with children older than 16 years old with a mental capacity of our age range)
3.  Did not receive help from another organization 
WHAT TO BRING WHEN YOU APPLY:
1. Social Security card of parent and children.
2. Photo I.D.
3. Proof of guardianship if not the parent.

Volunteers and donations

Toy donations are always accepted. For more information on how to volunteer or donate, please contact our office at 423-697-0031 between 10:00 am - 2:00 pm, Mondays - Fridays and thank you for your generosity.
